To obtain an Action For Animals HS License Plate:

• Email us and we would be happy to send you the AFA Specialty License Plate application.
• Fill out sections “A” and “E”
• Write check to “Action For Animals” for $64
  (The application mentions a fee. Do not pay this fee.
  The total cost of the license plate is $64 with a portion of
  the proceeds benefiting AFA).
• If you choose to do a Personalized Plate, please fill-out section “D”
• Personalized Plate Fee is $200
• Mail your completed application and check to:

Please allow at least 12 weeks for the state of Pennsylvania to process your application.
